nvrl e4062cc97b feat(fuse,core): number inodes by local id ...

A node's inode is now derived from its row's local id instead of a
per-run counter, so the same node keeps the same inode across restarts
and across every place that interns it. A lid handed out before the
row is stored reads as no node.
